Cereal

When the box mentions the nutritional information 'per serving', does that mean with or without milk?

Without unless it specifically says it includes X amount of a specific type of milk.

TOP TIP: Try using coconut milk; it has almost no calories, and adds a lovely flavour to breakfast cereal :-)

As mentioned above, it's without. Unless the box specifically lists milk. I use unsweetened vanilla almond milk in my cereal, which only has 30 calories.
