I worked out still when I had my hernia "umbilical hernia" to be precise. Didn't do anything to me adversely just be cautious and listen to your body. Probably planking would be better to do for core so your not scrunching your core up around the hernia (depending on what kind or where it is).

Yes calories in calories out but its not always that simple... Peoples bodies break down the Macro's differently. thats why Trial and error will give "you" what you need to be successful just record EVERYTHING so you have baseline on what works for you moving forward.

I don't know, something tells me this is dependent on insulin sensitivity. I feel like some people can get away with eating tons of "Carbs" while others benefit form a more low carb or Keto approach. But you will ultimately have to do your own body research. Trial-n-Error will answer almost all your questions lol
