Find a therapist that specializes in eating disorders. Why wouldn't fills work any more? Get a fill, make your pouch smaller = you will eat less. Your problem isn't recipes or portions, your problem is emotional eating. You said so yourself. Get thee to a therapist, seriously. :)

If you have a regular internist, start there. He/she will refer you to endo or gyno or whatever. When they tested you for thyroid issues, did they do any other tests? If not, demand it! Good luck.

Biking burns a LOT of calories. I used to take spinning classes when I was about your weight (I'm 5'3") and I wore a heart rate monitor that tracked calories.
