It may be a good idea to mix it up on some days. Perhaps you could do your 5x5 routine for raw strength, and mix in some 10 or 12x4 for some endurance. In either case, go to failure. Just a thought!

Check your calories, your calories burned using a heart rate monitor, and stop worrying about weight. Worry about body composition- lean body mass vs body fat. The number doesn't matter. I do Insanity and I lost a good bit of weight doing it. Just check yourself.

Get a Heart Rate Monitor to keep a check on your calories burned. Also, I do Insanity as does my girlfriend. I have lost weight rapidly whereas she has not. She lost about 8lbs the first round. Men just burn calories quicker. Just keep working. It takes TIME!

Typically yes. But still, the only real measurement is a good HRM. How else can you know how hard you are going? I recommend a Polar. I use one for P90x and Insanity.

Use a watch and chest heart rate monitor. That way you know! You cannot really measure it without one, because you might be working harder or not than everyone else.

The only way you would not be getting results from using P90x is if you are not eating right. Using a HRM is a good tool. I use one for P90x and Insanity. However, P90x is a very good workout. Plan your meals ahead of time. Measure your food and stick to it. Try to shoot for five meals throughout the day of about the same caloric amount. Weigh once a week and once a week only. Your weight constantly changes due to water weight, food, digestion, etc. You should weigh early in the morning once a week on the same day when you get out of bed. That gives you a fairly accurate weekly progress report if you will.

I'm with Hiker. I do Insanity and P90X, P90X+, and the 1-on-1's. Works great. Getting your heart rate up and keeping it in your zone for longer periods of time boosts your results and improves performance over time. Workouts alone aren't the answer though. Dialing in nutrition is even more important.
