He whispered it, but it was never revealed. I always assumed he told Rick that Lori was pregnant. I've read the comics so I knew that was in store, but the way season 2 has played out so far, Rick hasn't mentioned the doctor at all.

I watched the end scene a second time. It was pretty unmistakable that he was going for the pistol that he had set on the bar top. Rick's reaction was just faster. It's unknown if he would have actually shot Rick, but you have to assume he would have. Rick did the right thing. It was a clean shoot as they say.

The newer series is good, but I really liked the original series from the 70's. I was able to get those disc's through netflix. I've played the z-man games version based on the comics with several people that that have never read the comics. Being familiar with the characters and locations in the comic is not really required to enjoy the game.
