You are 100% right. It is trial and error. I didn't lose at my desired rate for a full 3 months. That's when I went back and calculated my loss against logged calories and I got a much better idea of calories out. For some people, it is fairly easy. And if it ain't broke, don't fix it.

Are you on regular metformin or extended release? ER causes fewer GI side effects in many people. And the regular causes diarrhea as a common side effect. So either way, it might help to try the other one and see if that does the trick.

I've only seen it in teenage girls, none with any comorbidities. Generally, they seem to outgrow it over time. My point was, even in SYMPTOMATIC patients with bradycardia, salt does not treat the rate, it helps with the concurrent hypotension.

Sodium not so much. Someone with syncopal episodes due to a neurally mediated hypotension (heart rate doesn't increase despite low blood pressure) may need more sodium to deal with blood pressure, but that is independent of rate.
