A bit of both here. I do eat out occasionally so I do need some questimates. I do try to break it down to it's parts unless there is a known good value in MFP. In class we have been through the '4 oz is as big as a deck of cards' discussion so I should be pretty close. When I'm home, I do measure most everything.

I used to feel uncomfortable at the gym or at running events until I just did it. Yes, there are more athletic people and try as I might, I'll never get close to a 14:00 5k. I had a similar experience even though I run predominately on a treadmill indoors. As the first person that responded said, there are studies that show these can make you hungry. Other than that, I still drink it occasionally. I think it's more about the amount and frequency than anything else. Predominately, I drink water but don't avoid caffiene like it's evil, but I don't drink it all day long anymore…

Depending on the style of restaurant, you can put together a very healthy meal. Just make sure to stay away from the calorie dense foods and get a good variety of the low cal (within your limit) foods.
