I'd go with the one on a flat surface, unless the scale is old then that could cause the scale to give you the wrong weight. I don't trust scales when they are on carpet, there is too much chance for my weight to be unbalanced when I am standing on a scale on it.

I think if the product says you can use it as a substitute then you can, but I don't like the idea. If the product tells you not to substitute it for meals then you should not. Howver, if you are running late the protein shake is better than nothing.

Calories are different depending on where you are getting them from. Such as calories from beer affect you different that calories from eggs, but it is not going to ruin your diet if you drink.

You look like you have a healthy weight, maybe drop 5-10 pounds since you said you don't like how you look, but thats it. You look good the way you are now so I think whatever size you are works well for you.
