Cup sizes are based on the difference in measurement between the band measurement and the fullest portion of the bust. One inch = one cup. So, 36 D would have approximately 40" bust, maybe 39 depending on shape and distribution. 34D is one full cup size smaller than 36D. 34D would have a measurement of 37-38.

There's nothing wrong with weighing yourself daily. I'll bet if you had been weighing yourself daily you would have seen more fluctuations and would not be surprised by the 138.5 reading today. Also, were these weights taken at the same time of day? You will weigh more in the evening than in the morning, usually 2-3 pounds.
