I am in the middle of season 2 of Breaking Bad (too hot and humid for me to run outside). But do what you enjoy--if you don't like steady state cardio then don't do it. You won't keep it up. The best exercise that you can do is one that you WILL do and have fun doing it.

"Hurt like heck going in" <---THIS is the reason why it isn't recommended for women who have not had a child. It is not a contraindication, it is because it hurts like hell! Those of us who have had kids have already had the hurt like hell part, so the insertion isn't as bad.

I haven't had one in ages-they typically plague me when I am running uncomfortably fast, long, or if I ate or drank too much before I run. I had them a lot when I first started running. Perhaps they resolve as one's conditioning improves? How long have you been running and are you in good shape or is this new for you?

Training for a long distance race has made my weight loss stall--I get so hungry, especially after the long runs! The last 10 lbs are hard, harder because you have to eat less because your body is moving less weight around. Have to rack up the exercise!

I think it must be relative to what your training is, I don't think there is a standard used for races. Living and training in Florida, I consider an overpass hilly, but someone living in the Sierra Nevada or Rocky Mountains would scoff at me.
