What was really painful and aggravating to watch was the way she reacted when the doctor told her outright that she was to STOP feeding him so much and to focus on helping him lose the weight. She got extremely annoyed by the doctor and stalked out of the room.

That's similar to a smoker's attitude about his or her smoking: I know I smoke too much, I know it's not good for me, but I don't care because the pleasure of it outweighs the difficulty and agony of having to stop doing it. Working out and exercising takes more effort and dedication than simply "not caring".
