Whats 'eating' clean' involve ? Mostly buying into misconceptions and pseudo-science. And also flushing cash down the toilet. Tiny germs of truth covered in layers of confusion. Better to look in to and understand IIFYM.

My doctor says nothing but lots of time (upwards of 2 yrs?) and really great genes will make my loose skin "shrink up". I lift heavy and have great abs, but it's likely no one will ever see them! So I hear you, but think this P-H idea is likely a gigantic waste of money.

I don't log exactly what weights I'm pulling or how many reps/sets, but I do log my NROL burn as well as cardio. I don't care how it affects my diary numbers, I just like to see my work in black and white.

Unless you'll otherwise be weighing in naked, it doesn't make sense to change the number based on the fact that you're wearing clothes today. Or do you plan to always deduct 2 lbs? And, if so, what's the point?

I'm also an asthmatic runner. When I'm feeling low in any way, I take time off. You won't lose that endurance unless you takes LOTS of time away. A week will not hurt you!

Absolutely. Sometimes I think I'll always be fat in my head. One thing that comes to mind is body shapers. I wore them for so long that I feel like I left home without my bra (that old nightmare!) if I go without one, though I clearly don't need them anymore. It is extremely doubtful that you are actually going over your protein needs, and DO ignore what MFP says you should be getting. For example, I'm 5'5" and 157, 35 yo and lift every other day. My daily protein goal is 165, which I find very hard to hit without supplementation, and I'm a big meat eater too.

Add protein powder to your oatmeal, DONE! I also add peanut butter or pb2, flax, nuts, almond milk, etc to balance it out. Now that's a meal.

Wear an HRM to get a general idea of your burn, though it's not accurate for strength. Or, just use mfp's suggestion for strength training. I don't recommend eating back the calories anyway, so it really doesn't matter for most people.
