6'4" Started: Oct. 2010 @ 294lbs Started MFP in April 2011 @ around 265 Current: 205 with a bf% of 9.4 Actually got down to 195 at one point but then started lifting heavier weights and adding calories. The 10lbs Ive added is fairly lean body mass that Im very happy with.

Lower your body fat %. Its the only way.

Depends on what the cause of the back pain is. If its a muscle problem, then probably not. I use mine daily and it relieves much of the pain/pressure in the lower part of my back. I had surgery to repair a ruptured disk a little over a year ago.
