That's not how it's recorded in my medical records, or my husband's, or my parents'. That method would seem to make sense, especially in the case of someone like OP with very high HDL, but my medical records use the calculation JustJennie posted.

I customized my macro settings: minimum of 130g protein and 68g fat. The rest is carbs, or more fat/protein. I normally eat 80-100g of delicious fat a day.

That's likely your culprit right there. Track your calories as accurately as possible. Weigh and measure anything that can be weighed and measured, and log every bit of food and calorie-containing drink that you consume. Do this for a month. THEN, if you're still in a "plateau," open your diary and ask for help again.

That, to me, indicates 100g cooked. I'd log 180g of delicious, delicious pork belly. To answer your question: generally, I always weigh raw, except when it's something like bacon or, as the case may be, pork belly.

I don't pay attention to net calories (never did). My maintenance is 2500-2600. I eat a slight deficit from that on the weekdays, surplus on the weekends.

This is true. Greek yogurt is typically made from straining regular yogurt to remove the whey, resulting in a thicker product with more protein per ounce. General Mills just added a thickener to its yogurt to create the "greek" style. For them, I believe it's a "milk protein concentrate" of some kind.
