I used to go on a week-long camping trip every year. No Diet Coke. I'd have horrible headaches for about 3 days and then I'd be free. Of course as soon as I got back home I'd get back on my fix again. :) It was the caffeine.

Totally agree. Generally if you aren't losing, it's time to crank down the caloric intake. There are some exceptions, but I suspect the biggest issue is incorrect logging of food causing more calories to actually be eaten than is being logged.

During the week I definitely pre-log breakfast and lunch and snacks, as they are all brought with me to work. I don't know what we will be having for dinner, but by pre-logging I know what is available to me at night calorie-wise.

You are basically on maintenance within a window of 10 pounds for the last 18 months. You need to eat fewer calories (or exercise more). Careful logging is the key.

Probably because so many people throw that term around with the intent of shaming. It's often leveled at people who fail at weight loss with no regard for the biological mechanisms that work against weight loss. As if the decision to fail was made completely arbitrarily for no reason.
