Consistency is key! But why is it so hard to maintain?

Hey ladies! A few of you already brought this up - you know what to do, you have all the tools you need, but you still struggle with sticking to your game plan towards healthier living. I'm no different. Here I am, a critical care nurse who, on a regular basis, sees how poor diets and inactivity negatively impact health, but I still have a hard time staying on track.
What do you do to stay on track? How do you stay motivated when your mind/body wants to take the day off (or three weeks, for that matter)? Does it ever get easier?
