Do you back off the weights every now and again? By how much, and how often?

Apologies if this is covered elsewhere, I did look at the FAQS...

    i think deloads are built in if you're going strictly by the stronglifts idea. something like 'if you fail x weight after y attempts at it (in separate workouts), deload z% and start working forward from that point again.'

    i don't follow to that extent, so for me 'deload' really just means that on this day i'm lifting a lesser weight than i was able to lift the last time. i have rheumatoid arthritis, so my weights can fluctuate quite a bit and the idea of 'deload' has a lot less meaning or bearing for me.
