New to group - Restarting SL - looking for form feedback

Hi! First, I've been reading this page for a year without joining and want to give a big thank you to all contributors. I've learned a lot reading these discussions.
The backstory: I started SL for the first time Feb 2015. Made steady gains for 3 months, then took time off due to the return of an old shoulder issue (left rotator cuff, impingement syndrome) and a hiking fall that banged my knee up around that same time - took a couple months off, did some physical therapy, resumed, and somewhat maintained thru fall, but then recently again took a couple months off due to 1) recurring right hip flexor pain with squats and 2) a midback rib/vertebrae alignment issue. Ugh. See photo of my all-time SL graph at the end of this post. (oh and a note: I stopped doing OHP and did an incline shoulder press w dumbbells for a while at the suggestion of my physical therapist - so that part of the graph is wonky.)
Now I am healthy and ready to restart - I deloaded to the bar (Squat, Bench, Row, OHP) or bar + 45s (DL). Could anyone offer some feedback on my form, please? I uploaded videos of each lift to a google drive folder - you can download and view.
