So, last month, we had a new patient visit with a 452lb diabetic 32 year old man. His a1c was pretty good at 8.0 - despite what we expected.

Yesterday he followed up. I grab him from the lobby and ask how he is doing. He says he hopes he's lost some weight. Internally, I do a bit of an eye roll. They always say that. We're community health, family practice. They never lose any weight.

27 lbs. I just stared at the scale and said "shut up" like 8x. When I asked him what changed, he said he didn't want to die. After rooming him, I went and asked the NP I work with, what did you say to him? He responded "I told him he was gonna die".

Any way. I just thought it was cool that we actually got through to this guy. 27lbs in 32 days. Now, I know at 400+lbs its easy to lose that fast but still! And his fasting blood sugars have been in the 90s, with dinner time highs at 140. It's so cool when they actually listen, and step up.
