I'm in the same boat, I do really well and then I fall off the wagon. The latest occurrence was in Sept. 2016. It was around that time I got the flu for a couple days. I didn't feel like eating much less logging. My 240ish day streak on here came to an abrupt halt and along with it the logging and exercising. I used to exercise 4-5 times per week, now I'm lucky to get in 3x/wk. I tried out another app (Sparkpeople) and found that I like MFP better so I recreated my account and here I am.
As far as exercising goes, I like to run and do yoga (those are my go-to's). But, I feel I've fallen into a rut and that's part of the issue with my declining workouts every week. I'm thinking of doing the free month of Daily Burn. I hope a meal plan comes with it. I try to eat healthy, though sometimes I still need a little help.
