What is your goal (for the 12 weeks and beyond)?

When I signed up for my challenge, I was asked how much I wanted to lose. I wasn't sure if I was supposed to answer for just the 12 week challenge or my REAL goal. I don't know about you guys, but I need several short-term goals that lead to my long-term goal, otherwise I will feel defeated easily.

How do you set your goals? Do you look at measurements, pounds lost/gained, % body fat, etc?
What is your 12 week goal?
What is your long term goal?
Any tips to stay focused?
