I have been having trouble with my hip flexor on my left side so I had to deload so it is not hurting. I stretched really well and it felt a lot better yesterday with less weight. Any advice on form? I feel like I am doing something wrong but I cant figure it out :)

It's difficult to tell just from the video but it doesn't look like you have a lot of hip drive. Another thing that's difficult to tell (since the camera is a little shaky) is if the bar is travelling in a vertical line - it doesn't quite look like it to me, it looks like it's coming forward a little over your feet. So maybe watch out for both of those?

I agree with both points of the above; the bar looks like it's going past your knees on some of your reps, and it should be over the middle of your feet. You also seem to start your squat and get out of your squat in two movements; you fold over first, then squat down, and coming back up, you do the reverse. I think some of that can be helped by improving your hip drive (mine sucks too, so I don't have a ton of pointers on how to improve hip drive), but it could also help by thinking about starting your movement with your hips/*kitten*/lower back. Rippetoe has a good video that talks about where a squat should start.

I have been watching this video series (Squat Rx) for stretching and practice for my squat form. It has helped me a lot. I still can't squat with a bar but its due to my lack of flexibility. This video is Squat Rx #1 and there are several more that follow this one. The You Tube video will link you to the others. It looks to me like you're doing the "good morning" which is what I do or did when I try them with the bar. Your form is still better than mine!

I agree with the above but your depth is great!

I started a thread last week about the hip hinge....I think that would really help you in regards to your hip drive and the movement.
