Need all your help please..

I am 26 / 5'11 and have been constantly battling with my weight problems since i was 12. I lost 40 pounds through diet and cardio mainly from 220 to 180 in 2007 and have been maintaining pretty well till now with weights 3 times a week, but i can't keep it off permanently. I go through 6 months of lean period to 6 months of weight gain if i am not very careful with my diet. So, when i realized that i tend to put on weight easily, i thought of making a permanent lifestyle change instead of going on a temporary diet. I tried calorie deficit approach and didn't lost much weight, then in desperation i tried psmf in december and lost 20 pounds in 1 month. Then my dad passed away in feb with whom i was very close to. He was my idol i used to look up to him in every way and i just finished my medical school to become a doctor. He was a doctor himself. So it was a huge blow for me as i lost him. After feb, i didn't have much motivation to work out or diet correctly. I still watched what i ate but was not very careful like i used to be for these 4 months and hence put on weight again. i am 191 lbs today. I want to get back in shape and don't want to do anymore yo yo dieting or any of those fad diets. i want to feel good, confident, and healthy throughout my life. I started keto diet 3 weeks back, i feel leaner but i don't know if i have lost much weight. i attached some pictures but i need some assurance if i am on the right track.

My keto diet:

Morning :
1 scoop Protein shake 100 cals 1carb
1 cup almond milk 35 cals 1carb
1 scoop almond butter 90 cals 2 carb

Afternoon:
2 Eggs 150 cals 1 carb
50 grams mushrooms
Broccoli 50 cals
1tbsp olive oil
4 chicken sausages 80 calories 5 gms carbs

Evening:
Green tea

Night:
1 Egg with 1 egg white omelet
Spinach 50 cals
Cream cheese 50 cals
Peanut Butter 1tbs 80 2carbs
Cottage Cheese 2 tbs 20 cals

i've been following this diet since 3 weeks and i don't know what i'm doing is even considered keto, even though i am eating <30g carbs with 1000 calorie deficit, i don't really know if i lost much weight these past 3 weeks. I'm 192 lbs now and will see for another 2 weeks if i am losing weight or i may switch to psmf. i am on the verge of starting PSMf this monday as i am desperate to lose weight, i can't deal with weight again as i am very conscious. Also, i'm not working out right now due to my preparation for residency board exams (usmle - exams after med school to be able to practice medicine in US) which i'll be taking in 3 months, so i won't be able to devote any time for gym ....I read up on different diets like atkins, keto, psmf, velocity but i want something which gives me good results and is sustained as a lifestyle where i can continue effortlessly without having rebounds. I have the dedication and willpower and i just need to be guided properly.

Thank you so much..

    hi,
I think you may be eating to few calories, and this is coming from someone who eats 1200 per day. I actually hate when people say that, but I'm saying it because you've been eating so few calories for so long. As a doc, I'm assuming you are on your feet most of the day, therefore you probably need 1500 - 1800 calories per day. If you eat to few, you set yourself up for binging in a moment of weakness.

I'm very sorry for the loss of your father and can easily understand the depression and loss of focus during this time for you.

Your ticker says you only have 20lb to lose, as such, a 1000 calorie deficit is way too much. You say you want a sustainable lifestyle, but after only 3 weeks, you're threatening to do a PSMF and you're putting stupid, arbitrary lines on seeing weight loss.

    Stop.

    Take a deep breath.

Now, throw away the scale. It's not doing you any favors right now, and is doing you more harm than good.

Next, track your food and make sure you're getting enough food and enough of the right nutrients. If you want to do keto, the do keto. As it stands, your intake looks rather protein heavy. That's not keto. Keto requires only enough protein to meet the body's needs for it (usually about 1g/lb of lean body mass). The rest gets filled in with fat. This means that your fat grams will be upwards of 150g and sometimes more. That's a good thing.

Even if you don't want to do keto, still go with the 1g/lb measurement and fill in with carbs and fat as you see fit. Dropping carbs from a SAD lifestyle and making up for it with protein is not only unnecessary, but un-ideal and prone to nutrient deficiencies at best (due to insufficient fat intake) and dangerous and even deadly at worst. Don't destroy your body.

Eat more food. I cannot stress this enough. The list you gave adds up to about 900 calories. That's too few for a sedentary 4'10" woman, let alone a man who's likely on his feet all day and is just south of six foot. You are damaging your body by starving it. Stop it. At your numbers, you should be able to maintain on about 3000 calories, depending on the specific activity level of your job. You're not even feeding your body's base metabolism, let alone your daily life and workouts.

Are you following a lifting program like StrongLifts, Starting Strength, or New Rules of Lifting? If not, start. You'll get better results following some kind of lifting program. Otherwise, you risk falling into a rut and not progressing, which doesn't do you any good.

Measure body fat. You're a doctor, I'm sure you have access to calipers and the means to learn to use them properly, or access to someone who can use them properly. If not, you probably have access to other tools necessary to measure body fat. That is more important than the number on the scale.

You did not gain the weight over night, don't expect to lose it over night. Honestly, it saddens me that I have to tell someone with medical training this, but I hope it's just because you're too stuck in your own head, and hope that getting you out of your own head will help.

    Also, meet Staci:

Yes, I know she's female, and the effects of lifting are slightly different for men than for women, but pay attention to her weight -- it actually has gone up from lifting, but because she gained lean mass and lost fat, she's thinner. Like I said, the scale lies.

Finally, consider seeing a therapist. You seem to have some issues that stem from a distorted perception of yourself and your reality. Talk to someone who's qualified and try to get those worked out. You'll probably be a lot more successful, and a lot happier with yourself, if you do that.
