You need to learn to train yourself to see working out and eating right as a life style change, not a chore or something that requires effort. Until you do that, your negative mindset will make you feel this way about both of those things, quite regularly to be honest.

I feel like sex is important but it should never be a brick that makes up a relationship or makes it fall apart. Making love to your spouse is a bonus that both of them should/can enjoy - but a relationship can (and should imo) be able to survive without it should that be the case.

If you mean surgery.. I strongly advise against it. You will be on vitamins and other supplements for the rest of your life.. and there are many complications. From my personal point of view, no. The quality of life while "big" is not good, no matter what anyone says. I've heard mixed stories about joining slimming groups. People say the best part about it is having support from other people losing weight as you are, but those that have lost weight (that I knew) gained a lot of it back afterwards. It was just too hard to stick to long-term.
