Pain in front of the knee the bike seat is too low, pain behind the knee seat is too high. Use easier gears with higher cadence instead of mashing if you feel like your putting on too much strain.

Good question. I think a century ride on a bike is a hell of a lot easier than a marathon. On effort basis I'd have it as more than 4:1 cycling to running if you go by distance. I'd just go by perceived effort personally...too easy to debate all the other possibilities.

I've read the range of calorie burn per mile as being between 50-90% of the calorie burn of running a mile. You can increase the burn by walking uphill.

Edit to add: map my run was 0.25 miles fast by mile 5 tonight vs Garmin. Not sure if it's the iPhone or the app, but considerably off. I might try endomondo next time to test it.

Push-ups, pull-ups, body weight squats and lunges. You can do dips by putting two chairs together and putting your feet on a third or a bed. Resistance bands for most everything else. Add in your normal cord work.
