Just jump straight into it, starting first thing the next day! Try really hard at each meal to 'eat right', and try to fit in a workout. At the end of the day you will feel motivated by your success, and then you can try to do the same again the next day :)

Hey, maybe you should speak to a doctor/physician/personal trainer at the gym for exercises you can do that won't hurt your back or neck. But as somebody said above, with any diet and any exercise you can lose weight. You can't choose where you lose weight from, but if you lose weight your midsection will get smaller :)
