You could do a time trial to see if you are close to your goal. Do a 20 minute warmup, 3 x 1 mile with 1 minute recovery, as fast as you can while still maintaining an even pace. (Try to get close to the same time each mile.) Average of mile times +15 seconds is your estimated 5K pace. 10 minute cool down.

Do you cross train? Non weight bearing cross training could help your knee issues. You could make every run count with 3 hard training runs a week, the rest cross training. That should be at least two cardio workouts a week in addition to the strength training suggested above.

I read Starting Strength to say that those who are fat (his word) should maintain the same high calorie diet, but not more. In this context, one might not be really eating at a deficit, but just at less of a surplus.

Sounds like you are running about 5 days a week. Only do three hard runs a week. On the other days run easy, or better yet imo, crosstrain. How many days/miles a week are you running? What is your pace for long runs? You should not run faster than planned marathon pace +20 seconds per mile. Slower for the longest runs at first. It only indicates the level in the blood stream at the time of the test. Protein defecency is exceedingly rare absent starvation, I suspect the dr over reacted.

It will vary according to starting weight. Losing 30 could sound impressive, but it either means the person was very large to start with or they starved themselves and risked their health. Again, it's a questionable measure of success.

You should also ask how long it took to gain it back. Losing a lot of weight quickly is not a good idea.
