Just ask for water, or tea, or sparkling water, nobody will think it's weird for you to turn down soda, at gatherings I often turn down drinks completely because I'm not thirsty.

you really need to see a doctor, could be any number of things, all bad, like a tumor, hernia, cyst, etc. etc. it's definitely not fat because it doesn't distribute like that.
