Lipid panel update... sorta

So, I had my cholesterol checked 8/13/14 and the numbers weren't too "bad". (317lbs, down 58lbs)

Total: 172.7
Trig: 95.36
LDL: 154.72
HDL: 33.02

The LDL/HDL was a little worrisome, but I was continuing to lose weight, and my Trig/HDL ratio was 2.89. Again, not bad but could be better.

I had my cholesterol checked again on 11/13/14 as part of a physical for school. (280lbs, down 95lbs)

Total: 171
Trig: 79
LDL: 118
HDL: 37

While my total number barely moved, the more important markers were definitely improved. Pretty much a 37 point drop in LDL, a 4 point increase in HDL and a 15 point drop in my Triglycerides. Plus, my Trig/HDL ratio was 2.15!

So basically my triglycerides were pretty much excellent, my LDL was good but I wanted to get it under 100, and my HDL was improving, wanted to keep raising that.

Why am I posting this? Found a service that will. let me get my lipid panel drawn (including VLDL, not included in prior tests) for $29.99. It's been five more months (all keto) and 53 more lbs down. Going to have it done today and I have a burning curiosity to see the results.

    Remember, too, that there are two types of LDL. If you are having the bigger fluffier particles, a slightly higher number is not as bad as if they were the smaller more dense particles. I don't remember all the details on this, but it is a major difference. Hopefully your place can tell this difference.

    This is why I am happy that this test includes VLDL, which is very low density lipoprotein, aka the "bigger fluffier particles". Makes me hopeful they will actually measure LDL too, vs calculating.

    Those numbers look good, and they're all going in the right direction!

    Really, the only risk you're showing is with the low HDL, but as you mentioned, it's offset nicely with the low TG. Those are the two factors most directly influenced by a low-carb diet.

    VLDL is considered sort of a precursor to LDL. If you were to get a VAP test where they separate them all out, they'd list two types of VLDL and four types of LDL. Of the 4 types of LDL, 2 of them are considered small/dense, and 2 are considered large/buoyent.

    Yeah, if i was truly concerned about my LDL I might shell out for a VAP, but I'm not so much. Hoping to see decreased LDL and trigs(though my trigs were fine last time) and increased HDL.

    My understanding of the small/dense LDL is that they're largely a function of how the liver handles excess carb intake, so you almost certainly have the good pattern of large/fluffy LDL if you're low carb.

    I just buckled down and send my endocrinologist the full list of things outside of his standard tests that he wanted me to do... I'd been putting it off because I didn't want the headache of going through all my notes. Sigh. We'll see if he flags anything.... I should be doing my testing Monday or Tuesday and getting my results Friday...

    Update: Holy *kitten* hell Batman!!

    My cholesterol results came in. At 227 lbs, and 6 months on keto, these are my numbers:

    Total: 295 (up 124)
    Trig: 74 (down 5)
    LDL: 243 (up 125 after dropping nearly 37)
    HDL: 37 (no change at all)
    VLDL: 15 (apparently estimated from my trig, so useless)

    I am not real happy with these numbers. 5 months (and 53lbs) since the last test, my HDL *kitten* refuses to rise, despite my taking quality omega supplements and regular coconut oil consumption, my total number is way the *kitten* up, and best of all, my LDL is through the *kitten* roof.

    How did your diet change from November other than the coconut oil? Some people see their LDL go up with saturated fat intake. At that level, it's worth getting a VAP or NMR to better understand the nature of your LDL.

    I switched to keto about a month before my November check. So vlc this whole time, moderate protein, high fat with a good percentage (40ish maybe) saturated. I eat eggs practically everyday.

    Have you read cholesterol clarity? Might help put the numbers into perspective, tho I understand it is disappointing Hdl hasn't risen further.

    For me, it helped to understand why high LDL is a risk. Peter Attia explains it well here:

    He goes into a lot of detail, but the bottom line is that the risk comes from LDL penetrating the artery walls. The more LDL particles you're exposed to, the higher the risk of penetration -- that's why age (time) is the biggest factor.

    Other the time, the key factor is the NUMBER of particles. The test you took measures the CONCENTRATION, not the number. So the increase in concentration can either be due to an increase in number or an increase in size. If the number didn't increase, your risk didn't increase.
