underweight but skinny fat- advice?

I’m underweight and trying to reach a healthy weight with the help of my doctor (who has me on a plan of no exercise yet eating 2,240 cals to gain 0.5kg a week) but i’ve lost muscle and have gained more fat. I have basically no muscle and i’m not allowed to exercise so i cannot even lift weights. as i continue to eat lots of food& gain lots of fat i now want to loose fat and gain muscle but how do i do this on a “no exercise” plan? I’m becoming more “skinny fat” and do not know how to loose the fat & replace it with muscle since i’m still too underweight. Shall i just continue to gain fat until i’m at a healthy weight (although by then i will be extremely skinny fat)? once i do reach a healthy weight what shall i do to become lean and not skinny fat? thank you for any advice!

Your doctor is likely concerned about the impact to your heart if you are underweight and trying to exercise. I'm sure he will clear you to exercise when you are a healthy weight. In the interim, discuss with him the possibility of very low impact exercise (like slow walking) if you are concerned about the way you feel with no activity. He might clear you for limited amounts of low intensity exercise if you are able to maintain a sufficient level of food consumption.
In any case, you should follow your doctor's orders in the matter because being underweight is quite traumatic for your body and he is looking out for your long term health.0
