"Off in the Wilderness" 'Long Post'

Since around December 2020 I've been checking in and weighing daily and participating with some post but for the most part have been off in the wilderness since at least January 1st of this year. DW and I together during this time have been struggling with a family issue. It's not going to be over for quite awhile at least a year if not more. We will get through it.
I had succeeded by by the end of April to have approached my wits end. So after discussions with my nutritionist late April she asked would I be open to a referral to an MD specializing in weight loss so DW and I agreed because over the past few years her weight had crept up about 50 lbs as well.
Since January 1st to Wednesday May 19th I manage to have gained over 30 + lbs. From my low after my surgeries and other procedural health issues 6 all in total in 2020 and gained over 40 lbs last year sometime.
So I made the appointment for May 12th with a clinic here that specializes in Bariatric surgery and non Bariatric weight loss. I have never been a fan of weight loss surgery because without a behavioral change surgery wouldn't help things. So naturally being around WW for years and then changing over to kcal Tracking ect; I opened the door and my mind to their program.
After the blood work and consultation their weight loss plan was a meal replacement shake for breakfast and lunch and a small plate considering right now of meat/protein 3oz and 2 side non starchy vegetables 1/2 cup each. At least 64 oz of water daily. No snacks No nothing besides what I've described except multi vitamins and other supplements we've been taking.
I've been down this road before with good healthy meal replacement shakes and other low carb diets. Since beginning
the shakes and diet last Thursday 5/20 I've lost from my high on May 12th of 337lbs w/clothes to 323.2 lbs so in 9 days I've lost 13.8 lbs. So now I've leveled off @ 320+- for the last couple of days. I told DW that after the body is shocked (my term) then the work starts with the ketosis. I'm really not sure how long we can stand being on the meal replacement program and protein/2 veggies. I've been hungry everyday and feel deprived which is normal for these type of programs. So I've been asked the question before more than once what are you going to do different this time.
My goal is to use this program as a jump start to a behavioral change and habit along with accountability which can last the rest of my lifetime. Their goal and I agree is 1-2lbs a week. Sticking with the healthy choices and get back to 200 lbs along with a get start moving program doing something at least 3,4,5 times a week preferably walking. The accountability portion of this plan is very similar to other programs it just has a Doctor attached to it. That doesn't mean they know anymore about each individual's weight loss except the numbers and averages are on their side and access to additional research for sure.
If your reading this you've probably been following along this journey with me since 2010 as I have been following many others a lot of which have moved on to who knows where.
So the reason for this post/blog was to help get some things off my chest and get my head back in the game. The Family issue will still be a mental hurtle as we continue forward.

I wish you the best for your results. I’m struggling more mentally than ever in my lifetime. I was too afraid to try WLS in 2012. WW works but I have strayed far from The Plan.

Getting your head in the game is the most important step. In fact, I’d say it’s mandatory for success. After all, if your head is in the right place, your butt will just have to follow.1

Derrick, I have always enjoyed your posts and have noted the various medical and personal/social issues you've struggled with over the years. It's been a challenging road for you for sure, and it's not a surprise that you feel that your off in the wilderness.
As noted elsewhere and at other times, weightloss doesn't really work until one is in a mental place to lose weight. Perhaps you are beginning to see yourself as hitting "rock-bottom," and thus ready for a change? Sometimes we need help, in terms of other approaches (be they WW, other nutritionists, clinics, surgery, or whatever). I think you'll have to decide for yourself whether your in the right head-space yet or not. My only concern is that if you're not in the head-space yet, then surgery will just be a risky and costly temporary measure. If you are in the right head-space, then maybe it will be just the jump-start you need.
