Anything you eat that looks different on your plate than in nature has been processed in some way; however, an ultra-processed diet comes with health risks.
Eating lots of ultra-processed foods has been linked to increased rates of hypertension, diabetes, heart disease, obesity, and dementia.
Pay attention to the ingredient list, and look for minimally processed foods with whole ingredients.

Oops. More and more articles seem to be paywalled these days. I used to be able to read a few free articles on the Bicycling magazine website. Not now. I was able to sign up for a $20 yearly subscription, which is okay because I'll get my money's worth.0
