Very First Endo Appointment - What questions should I ask?

After waiting for an appointment since February.... I finally have my appointment this afternoon. My symptoms are hypo/hyper active thyroid (it spikes and drops about every two months), excessive mood swings and changes in energy, weight gain/inability to lose even with the help of a nutritionist and PT who both say I am doing everything right, itchy skin, severe menstrual cycle, multiple vitamin deficiencies, ridged nails.
The list goes on.
Any advice for questions I should be taking a long with me, would be greatly appreciated.

I think you have all the info the consultant will need. If you do not understand anything as the consultation goes along say so, at the time do not try to think of it later. Show you are interested not just going through the motions I am sure questions will present themselves. If it seems right to say my nutritionist or personal trainer that but don't feel obliged to. I hope the blood test which will need to be taken puts you in easily diagnosed situation, unfortunately this might take time to medicate. After this consultation have a sheet of paper or a note pad to jot down anything that comes to mind. You could make notes while you are there but only if relevant. I hope you will not need to wait too long for the next phase.

I have a blood test a few days before my appointment. My endocrinologist explains my results, I detail my symptoms, then he adjusts my medications appropriately.
