Sensation of blacking out when squatting

Since starting Wendler 5/3/1 my husband has started to feel like he's going to black out (blood pressure changes?) once he reaches are certain amount of weight/reps squatting. We were doing the high rep lower weight squats as assistance - so we'd do the pyramided sets and sometimes he'd make it through the last set ok and sometimes he'd start feeling it then, and then around rep 6 or 7 of the lower weight set he'd start to get it for sure.

I have no idea if its relevant but his fasting BP mid day was 110/70 (or are those flipped....) which I think is normalish.

Thoughts?

    Is he breathing properly during his sets? If the weight is heavy enough for him, he could be concentrating so hard on his lift and not realizing he isn't breathing well.

    Could he have blood sugar issues too? Maybe he isn't getting the right food/amount of food before the workout.

    First thought that came to my mind was his breathing as well. Is he straining and holding his breath?

    I'll get this feeling when I'm done and exhale. One has to somewhat hold their breath during the lift to keep their core tight. No breathing inbetween reps will probably result in the light headiness

    Now if I could get him back to the gym with me I could test the breathing theory
