I am looking into leaving stronglifts behind and moving on to a new program. Is there any special "transition" phase to do before moving on, or do I just finish up the week with stronglifts and start anew Monday? I haven't read about this anywhere and I was hoping you guys could shed some light on this topic for me. Thanks in advance.

IMO, you do not need any transition to go from one program to the other. If you have not had a training break for a while you *may* want to take a week off just because the timing works, but other than that, I would not do anything special.0

There's usually no need for a transition. Most of the well-designed programs are similar enough that transitioning from one to another is easy.
The only way I would see a need for a transition is if you were doing a program where you needed to take a week to find your 1rm on a few lifts, or if you were doing something like HST that calls for "strategic deconditioning."0 -
