Brian Shaw's BMI was 45.4 when his weight was at 412lbs. He isn't representative of that weight range among the general populace, but he and his competitors also clearly show that "fit" is something that can certainly be achieved in the absence of underlying medical issues.

Ashley Graham is also quite beautiful. Couple that with however her weight/health is perceived and she's almost the perfect target for media vultures. I sure wouldn't call her obese though.

Take one mouthful from stove. Sit and eat it. Return to stove for the next mouthful. Sit and eat it. Slows you down and increases NEAT> Win Win! Heck I don't know! - I'm the same way and my wife eats like a turtle too lol. A conscious decision to slow down is really all I can think of. I just haven't tried it yet.

RE the bold, very much so for me. Maintaining without counting is extremely easy for me using a "loose" IF. I don't count to the minute and will vary as much as a couple of hours on some days, but that window is always in the back of my mind.
