From looking at your height, weight, activity level and glancing at your diary, my best guess would be that you are not eating enough. And not near enough protein.

How is your diet? As everyone says "you can't out train a bad diet". I'm learning that the hard way too!. Have you tried HIIT? I lift weights 4 days a week and do cardio 3-4 times a week but I keep it under 2 hours a week. I would recommend HIIT cardio, weight training and a good, calorie deficit diet. Good luck!
