When I splurge, it's common for me to eat 5,000 calories in a sitting.

I think that you shouldn't try to change everything in one go. Focus on losing weight or quitting smoking then, once you have one under control, attempt the other. However, feel free to ignore my advice since quitting smoking hardly helped me.

Most of the jobs I want to do when I am older require health checks so I absolutely cannot allow any health care providers or future employers to know about any addictions, depression, alcoholism, lack of motivation, or anything that is bad.
