I always try to stay under my calorie goal. However, there are some days where I'm hungrier and eating more just can't be avoided. Some days I'm less hungry. It all averages out in the end.

I wouldn't even look into surgery. If anything, I would consult a nutritionist. Maybe they have one at the clinic your doctor referred you to? Fruit is healthy, but it sounds like you need more balance. Maybe you need more vegetables? Suggestions: Hard boiled eggs. Salads, esp with chicken or fish added Celery and peanut butter or cream cheese Veggies and dip

Well if it's happening less than once/month, it's not going to make any difference to your overall progress. I would suggest just make your best estimate and move on.

You can start by adding 200-500 calories to what it gives you. From there, it'll be a case of trial and error. If you find yourself not losing after a couple weeks, then it's time to decrease your calories.

Come prepared. You don't have to drink water. Bring something fun like diet pop or somethign else that won't cost too many calories. Perhaps you can mix your drinks with diet gingerale?

My guess is they will come off when you lose weight. There is, however, a medical condition where you get man boobs even if you're not overweight. You don't need to buy anything fancy. Eggs are good. Buy big bags of rice. In-season vegetables and fruit. If you eat beans, you can save money by buying dried beans and soaking them overnight before cooking. I calculated it once and buying dried beans saves about 50%.

I wouldn't bother logging it, personally. However, I bet your dentist could be concerned about you chewing that much gum. Yes, it can be good for curbing your appetite, but it can really wear your teeth down.
