I run 5-6 days a week, each day is different mileage, but when not training I am at ~50 miles per week, when training the average is closer to 65 per week.

Either underlying back issue or more likely, if you don't have back issues otherwise, running form gets bad as your core breaks down when you approach the limits of your muscle-skelatal endurance (not the quitting part, the pain part.) Core work and slowing down and building distance slowly while maintaining form can help.

Vinegar works great, I just through 1/2 cup into the laundry with the clothes and then whatever detergent we happen to have. I also do not machine dry the cloths as this seems to set the stink in.
