I have moderate-severe GERD (I take protonix twice a day).
I frequently have trouble determining if I'm truly hungry, thirsty, or if I've got some heartburn acting up.
Any tips/advice?

I don't suffer from GERD myself, so hopefully someone more knowledgeable comes a long soon
I can help a bit with the thirst though.
If you are pro-active with drinking your water, it should hopefully mean that thirst isn't a problem. I usually have a 600 ml (1 pint, 20 oz) bottle of water within reach of me at all times that I fill up whenever it's empty again, and that stops me from feeling thirsty. Over the winter I've been drinking a lot of herbal tea as well, when the water is too cold for me.
A simple way of testing whether your body is asking for food or liquid is to drink a glass of water first. If that doesn't help then eat something.0
