how often do you tend to mix up or change your fitness routine?

I know it's good to keep the body guessing (and the mind too!) and it keeps me motivated in the gym, but obivously you want to follow a programme for long enough to get results.

i weight train and mix in cardio and at the moment change roughly every 4 weeks around similar programmes but different exercise and sets etc

4 weeks is pretty good. It will keep you from hitting a plateau. I keep a journal and when I see two weeks go by and I'm not getting any stronger in an exercise then I change things up. I carefully choose my routine's and stick with them as long as I can. This is especially so since I've been having elbow issues. First it was some typical tendonitis and no sooner was that better then my elbow itself started hurting. Not a lot of options for me so I'm limited. Once it's better I'll probably change it up a bit more than usual to prevent losing out on a week or two of building.

You should change your routines when you get bored of them or no longer see gains.
