Anemia...from running?

I'm a 55 year old male who runs, more so this year than ever before. My iron levels have been falling a lot the past few months (hematocrit now at 30, hemoglobin at 10.0). Although it coincides with my increased running (and weight loss, too, by the way), I have a hard time imagining these are connected. As yet, the cause of my anemia is not diagnosed and the most likely causes are all unlikely. So I'm left wondering if "Runner's Anemia" is relevant. I can't imagine it, but in fact I often feel more tired if I run daily for several days in a row (e.g., I had to take 3 naps yesterday) but since I'm also dieting I wonder how much of that is from low blood sugar after morning runs. Also, I donate whole blood every 8 weeks and I'm sure that's not helping any and also confounds a diagnosis. I've stopped donating blood but continue to diet and exercise since I'm doubtful they are connected. So......
Does anyone have any personal experience with something like runner's anemia? Could it cause such a large loss of red blood cells to cause my hemoglobin level to fall that low?

I have many questions regarding this as well. I went through cancer last year, finished up chemo and radiation in November. I was slightly anemic in January when we did blood work. I was running and feeling pretty well. I ran over 60 miles in March and when April and May came I hit a wall. I could hardly breathe couldn't find the energy to run, my pace tanked and started getting nightly cramping in my calves. I went to my naturpathic dr and she did more blood work and my hemoglobin and hemocrit have dropped even more. Part of mine is due to the chemo and radiation but we also believe that my intake of vitamin B and Iron was not enough to accomodate for all my running. I got a vitamin B shot that helped a little but they suggested doing an IV this week to try and help. Would love to know if you find out any more about this.0

Could it also be from your change in diet?
Have you accidentally cut out too much iron?0 -
Anemia is from lack of good diet. I had it..started eating things like spinach,watermelon,prunes,cereals (iron fortified), raisins...stuff that were healthy and loaded with iron...then I was all better0

Anemia is either from a dietary deficiency or blood loss.
Running will not cause either of these but anemia can cause weakness especially with exertion.
Since December I have lost 12 pounds (am at goal weight) and I donated blood at the beginning of May.
I have only donated a couple times in my life and did fairly well those previous times but this last one really wiped me out for several days.
I added iron to my macros so I could keep track.
You may want to consider a multivitamin with iron and increasing your intake of iron rich foods to begin with.
If this doesn't help, go back to your doctor to be tested for hematologic disorders.0 -

I remain unimpressed by everything I've read about "runner's anemia". None of the popular claims appear to be substantiated by any evidence and so they remain as hypotheses presented as facts. The proponents should go into marketing or politics or modern "journalism". In my case, I now have a diagnosis for my anemia which involves a slow blood loss in my small intestines. If this is true, it should be resolved this month by cauterization during a small bowel enteroscopy, which I'm pleased about (although now I want to know why I have such a condition).
As to how running could have made my situation worse, there is one site where I read a possible explanation of runner's anemia is that it's the result of greater blood loss from the substantial and sustained jostling that a distance runner experiences if he or she has otherwise modest bleeding in the digestive tract. That sounds plausible and might be consistent with my experience. But it, too, appears to be unsubstantiated by any evidence that I can find. Why is it that despite all the medical testing available, we still appear to be living in the dark ages?0 -

There is an entity known as pseudoanemia that occurs in runners. This is due to expansion of the plasma volume (liquid part of the blood) as a result of training; this causes a lower percentage of the cellular components of blood.
There is a phenomenon known as heel strike hemolysis (misnomer, as this occurs in swimmers and other athletes) in which red cells break down, causing anemia. These are the main causes of anemia that are particular to athletes.
Gluten intolerance (celiac disease) and other dietary issues may cause anemia as may a wide variety of health conditions. Additional tests to determine the cause and chronicity of the anemia are advised; ferritin levels are frequently performed to assess iron stores.0 -
Well, OP, if I were you, I would at least set your diary to track iron intake instead of sugar (which is kind of useless) and take a look at how you've fared over the past 90 days compared to the RDA.
The vegetarian sources of iron mentioned above are great, but you need to eat plenty of foods rich in vitamin C for non-heme iron to be absorbed. If that is a problem for you, eat red meat.
Finally, I cannot imagine why anyone would let an anemic person donate blood.0 -

I can't find any evidence of it anywhere in medical journals or anywhere that refers to any real data. Actually, that explanation sounds a bit silly to me - an old wive's tale maybe? - and so maybe that makes me overly skeptical of all the explanations. It's not like I have a big red bruise on my heels when I run where the remains of red blood cells have died. I can't find anything like that described anywhere. So unless someone cites a medical journal with data, I remain skeptical about all the explanations.0

Alarm bells ringing here. 55 year old Western male, recent onset anaemia - needs urgent investigation. Please go and see your GP/family doctor (if you haven't already done so). You say that the most likely causes have been excluded - UK BSG guidelines include urgent endoscopy (top and tail ends I'm afraid), screening for coeliac disease and other blood tests as a bare minimum. Have these happened? If not please get your doctor on your case. Once everything has been excluded perhaps you can pin it down to running/extreme dieting or whatever, but at the moment I'd be dubious.0 -

I have anemia which is well controlled with iron supplements (prescribed by my dr) which is due to lupus and celiac disease. I would recommend seeing a physician for clarification of what the underlying issue is before you do self treatment.0

Yes. Upper and lower GI were both clean as was a CT scan. A "pill cam" showed possible AVM to be confirmed and cauterized during a small bowel enteroscopy soon. Thanks for your concern.0 -

WTF donating blood every 8 weeks.... and they're still allowing you to do that in spite of anaemia? I think that's the most likely cause of the anaemia!! Who are you donating blood to, why are they taking so much and why are they doing it in spite of you suffering from anaemia??
The UK blood transfusion service takes blood from donors 3x a year, there are rare circumstances when they might take more than that from a donor, but any sign of anaemia they won't take blood from you at all. In fact my mum's been told by them never to give blood again as she failed too many anaemia tests, and she's basically genetically predisposed to it and they consider it dangerous for her to give blood at all.
Really why are you even considering running as the cause when you're giving blood every 8 weeks....?? It won't be the running....!0 -
If you have a pre-existing condition such as spherocytosis (I have this) running can cause anemia, as your blood rushing through your veins from the vigorous exercise that you're doing causes your fragile blood cells to break down at a faster rate, and also be processed through your spleen (and destroyed there) at a faster rate. It is not very fun to realize that vigorous exercise makes you anemic, especially when you're trying to lose weight. However, you would probably already know if you had something like spherocytosis, as it is something that you're born with and it has other symptoms that you would have noticed long ago. So as far as I know (and I did quite a bit of looking around for something else because I didn't want my spherocytosis to be the reason I had to stop running) that is the only medical thing that can cause anemia from running. Other than poor diet. So maybe if you change your diet a bit that might help. Try eating a bit more?0

I have anaemia at the moment, like other posters have already commented I had to have an endoscope to rule out internal hidden blood loss, once I got the all clear from that and also the doctor had taken blood samples to check for any other underlying disorder I have been put on 200mg iron tablets 3 times a day for the foreseeable future. Its not for you to have to guess why you might be anaemic, your doctor should be getting to the root cause of it for you and treating it until youre better. In the meantime dont donate blood.0

My first shriek would be for bloods to be taken to rule out pernicious and/or megablastic anaemia. You are a little bit old to get this, usually hits in 30/40s, as I am only too aware.
If you do have pernicious then no dietary supplement will make a blind bit of difference as you won't have the Intrinsic Factor in your gut to process any of it.
But 8 weekly blood donation does sound excessive and may be involved.
I hope you get it sorted, quickly, but don't clutch at straws, keep pestering your doctor.0 -

I've often wondered who is being overly cautious or who overly risky. At least I've always been tested for hematocrit before every donation and I would assume that's a universal practice. I've donated at the maximum rate for 15 years and only now have had a problem which appears to be unrelated to my donating blood. I should add that I know many others who do the same with no apparent problem.
I should also chime in with some of you that anyone with symptoms of anemia should see their doctor. One of the few unanimous statements I've read many times is that if you're an adult male or non-menstruating female, the likelihood that you have internal bleeding in your digestive tract from colon cancer, gastric ulcers, or small bowel bleeding is well over 90%. Don't just shrug this off or attribute it to your diet (this is quite rare apparently) or exercise. Taking some iron pills would help you only put off the problem while you continue to bleed.0 -

blood donation = bleeding. That is a huge amount of iron and a huge amount of blood cells. the body can take it and replace it if it's not too often. But you've admitted to taking a ton of iron supplements to "pass" the blood donation test and keep donating... you're putting a big stress on your body by doing this. It's not just iron your body needs to replace that blood. Are you on this site to lose weight or gain muscle? Either of those goals is going to affect your body's ability to make new blood. Or goals related to succeeding at weightlifting or endurance sports.... You've basically pushed your body to the limit when it comes to making new blood. It's not making it as fast enough to replace the blood lost in donations. Even if your body coped in the past, you're older now.
Yes anaemia in men and non-menstruating women is rare, and usually the sign of a serious problem. In your case, if i was a betting person (I'm not, but if I was) then I'd bet the cause is donating too much blood. It's possible there's another factor as well, but seriously you want to stop giving blood for a long time.
As to who's over cautious... I know in the UK they'll in some circumstances accept donations at 8 week intervals but the reason they don't under normal circumstances is because people will give as much blood as they're allowed to and end up stressing their body to the point that they get ill from it. Seems like that's what you've done. Especially the bit about taking loads of iron tablets until you can pass the test... you should have taken that as a warning sign that maybe you need to give your body more time to recover.
I'm not a doctor but I have a background in biological sciences. Do get a doctor's opinion, definitely, because there may be another cause like a slow internal bleed, but really it sounds like you donated too much blood, and the transfusion services shouldn't, in my opinion, have let you do that.0 -
I just want to add that anyone who gives blood is amazing and a life saver.... no doubt about that. But you do need to keep an eye on your own health, and look after yourself, and look for warning signs that maybe you need to lay off donating for a while.
I also don't think they should just take blood every 8 weeks without further health checks. IMO the blood transfusion people failed in this case.0 -
I have iron deficiency anemia (my hemogloblin is fine, it's my ferretin, aka iron stores, that are rock bottom).
I really doubt that running is the cause of anemia. Lack of iron either means a) you are't getting enough in your diet or b) your losing your iron through blood loss (example. blood donations, mensturation, or internal bleeding).
Definitely stop donating blood! You cannot afford to lose iron at this point. I even stopped getting my period (I take birth control pills continuously, without a break) to prevent any sort of blood (read:iron) loss. I have endometriosis so I would lose a LOT of blood. I accidently forgot a couple pills and got my first "period" in almost a year last week... omg it tired me out soooo much!! I could not workout at ALL, could barely even do normal activities. So now I'm taking my prescription iron pills again. My diet alone is not enough to replenish my iron stores (ferretin).
I'd see a hemotologist rather than a naturopath at this point. You may need a serious iron supplement, not just a multivitamin. Take it with vitamin c, and AVOID calcium! (Many vitamin C supplements also have calcium as a non-medical ingredient, be careful!) Calcium binds to iron and prevents it from being absorbed by the body. Vitamin C on the other hand helps the body absorb it better.
Get heme iron in your diet. You get heme iron from meat, especially red meat. Non-heme iron sources like spinach and other veggies are much harder for your body to absorb. I used to be vegetarian, but now I need to eat meat occasionally (yuk), as well as take prescription iron pills (I take euro-fer pills, I use to take pallafer but the capsules were too big.)
EDIT: I totally overlooked the part that said this was a dude. Men with low iron are pretty rare.... You need a doctor! ASAP!0 -

I agree though that yours is lower than it ought to be.
I run a lot in GA in high heat and take an over the counter iron supplement nearly every day. My blood was tested last month: Red Blood Cell Count - 4.56 (low); Hemoglobin - 14.6 (low end of normal); and Hematocrit - 42.8 (low end of normal). We are about the same age.0 -

Scott, what I've read would say that if you, as an adult maale, have to take an iron supplement daily to maintain your hematocrit, you should see your doctor. The consensus medical opinion is that taking such supplements in a healthy male is unnecessary and should lead to a higher than normal iron level with toxic effects (mostly indicated by digestive upsets of one kind or another). It may be you have the types of problems I've mentioned earlier. Please give that serious consideration.
By the way, I did not take supplements to pass the blood donation hematocrits. I simply added red meat, spinach, lentils, thyme, beets, etc. to my diet for three days prior. After still failing the hematocrit again 8 weeks later, I saw a doctor by which time my hematocrit had cratered. My stupidity was in giving blood twice when I was barely passing the test.0
