The nutrition label on my package of frozen fish says that 113 grams of fish is a serving. Is this for frozen fish or cooked fish?

Rule of thumb is that the weight on the label refers to the product as packaged. Unless it states otherwise. So if the fish it raw in the package, that's the way it should be weighed unless the package says "cooked weight."0

Rule of thumb: "As packaged"
Unless it reads cooked weight, or something else indicating some sort of processing after removal from the package.0
