Need a great program/app for logging

Hello,
MFP is about the best I've seen for logging food, exercise, and weight/measurement loss.
I'm looking for something to suppliment this logging. I want to log the things I do every day with a check mark (or not checked.) Then at the end of a week or month, I want to run a report on the percent of time I was successful with those things.
So for example, every day I want to go to a screen and check off if I"ve done the following:

Stayed in calorie range
Gave myself 15 minutes of down-time
Drank all my water
Exercised
Read something motivational

Also, it would be helpful if I could collect my NSVs somewhere so I can see them all together on another screen.

Any suggestions on what you use for these two things would be helpful!
Thanks!!

In my opinion, your best bet would be a spreadsheet.

Good idea! I didn't think of a spreadsheet. Does Google Docs have a spreadsheet option so I can do it on the road if I'm not home? Just wondering out loud - I'll go research now.......

Wow! Google Forms is pretty wonderful nowadays! I made the check list I was looking for. Hopefully I'll be able to run reports off of it easily.

There is an app called TracknShare lite that could probably do what you want.
