Here's what I do for HIIT on a treadmill -

    Warm-up
    30 seconds all out (10 mph @ 7% incline)
1 minute slow jog (or walk) (4 mph @ 7% incline)
    ***Repeat 7 times***
    Cool-down

Adjust the work time, rest time, speed, and incline based on your fitness level.

If you're really doing HIIT, at the end of each work/sprint you should be sucking air and your HR should be just about at max. Rest should be just enough to let you get through another work/sprint session. By the time you're at cool down you should be ready to collapse (you probably should be thinking about throwing in the towel from work session #4 on).

I do similar, but 30/30 secs work/rest ratio at 13.8-14kmh 6% incline repeated 10x. Great workout.

I do mine on a spin bike with a heart rate monitor. 2-3 minute warmup, 30s all out and then active recovery for however long it takes to get to get my heart rate back down below 135. Usually ends up being 30:60 work:rest at the beginning and 30:80 by the end. 10-12 intervals. 5 minute cooldown.
