I've had back surgery.
Gained weight.
Always in back pain.

Welcome! Get your water in. Monitor your food more carefully because exercise is a limited option for now. The exercise gurus will have more ideas on exercises that are safe to do.

I've been doing physical intensive jobs for 7 years now. *kitten* up my back, ignored it, and it just kept getting worse. Now I struggle to get out of the bed in the morning. One thing that helps with my back pain is this herb called kratom.

As far as physical therapy, you HAVE to exercise your back and do lots of stretches. Start watching and doing yoga videos. I like to watch yogawithadriene on YouTube.
But really, the only way to fix your back is to use it. Don't over work it but start working on strengthening it. The kratom will help to do that since it's a pain reliever and anti inflammatory.
