You talk about eating healthy and controlling portion sizes, but are you counting calories? You lose weight from being in a calorie deficit.5

None of that matters.
How many calories are you eating and how do you measure them?
You can lose weight even if you're bed ridden2 -

You don't mention how many calories a day you you are eating, what your weekly deficit is, or if you are using a digital food scale to weigh all your food.
All the exercise in the world will not help you lose weight if you are not eating at a deficit.
No style of eating will help you lose weight if you are not eating at a deficit.
Plug your stats into MFP. Say you want to lose .5-1 lbs a week.
Buy a food scale and use it for everything, liquids, measuring cups and spoons.
Log your exercise and eat those calories, MFP expects you to. You may want to start by eating back 50-75% and adjusting after a month so you are losing the amount you set up MFP for.
Don't expect consistent week by week results, look for a long term decline in weight.

Until you put a number to all those descriptive words like eating healthy, portion controlled, you won't know how to improve going forward. Even if you are cooking your meal, you should take advantage of create your recipe function. Remember to log everything including oil even if they are olive oils, and all those bottled sauces. They can roll big number without you knowing.4

You cannot out train a bad diet.
You need to be in a calorie deficit to lose weigh. Weigh out everything you can (I don't weight out my leafy greens anymore - I use the eyeball method as if I'm off by 1/2 a cup, that's a whopping 3 calories). Buy a food scale and use it religiously!!! Do not use a measuring cup for solids, only liquids.1 -
You're eating too much. Simple as that.
Re-evaluate your caloric intake and ensure you're in a caloric deficit daily/weekly. Doing this your body has no choice but to lose weight.2 -
