Help Keeping Motivated

I’ve been trying to get healthier and lose weight since 2014... it’s now 2019 and I’ve gained about 25-30 lbs since 2014 and am no longer feeling good about myself. I’ve lost 10 lbs before m, gained it back, lost 5, gained 10 back... I can’t seem to keep motivated to keep healthy habits and continuing exercising. Any tips to stay motivated?
