I'm wondering what are the health risks of a 5'3" 20 year old guy who weights 85 lb? Not asking for myself!

Those stats give this person a BMI of 15.
Given that, they are likely to need serious professional help in order to safely address their weight and other issues.
This is not something that is suitable for a “friend” or a bunch of internet strangers to fix.8 -
In and of itself being underweight carries risks, but it's also relevant to look at the reasons the person is underweight (eating disorder, illness, ...).

Also, do you know how they are staying this thin.? Purging can lead to electrolyte imbalances.which can be serious in themselves. They need to see a doctor to find out why they are not gaining weight and most likely a specialist in eating disorders.1
